Nunavut Home and Continuing Care
NU Continuing Care
Nunavut's Home, Community, and Continuing Care program supports adults with disabilities and seniors who need care to continue living in their homes and communities. It provides home and community care services, continuing care, and support so Nunavummiut can care for themselves with help from family and community members.
Conseils utiles pour la demande
Contact the program early, before a crisis, since support can help you stay in your home and community. Involve family and community members, since the program is built around their care. Ask for an assessment to determine the right level of support, including home care, community services, and continuing care.
Qui peut être admissible
You may qualify if you are a Nunavut adult with a disability or a senior who needs care to continue living in your home and community. An assessment by the Territorial Home and Continuing Care Program determines the services and supports you need.
Documents requis
An assessment of your care needs by the Territorial Home and Continuing Care Program, and information about your living situation and family and community supports.
Comment présenter une demande
Contact the Territorial Home and Continuing Care Program to arrange an assessment of your care needs. A care coordinator helps identify the services and supports that would help you stay in your home and community.
Erreurs courantes à éviter
A common mistake is not seeking help until a crisis, since early support can prevent a move to institutional care. Another is not involving family and community members, since the program is built around family and community care. People also miss that an assessment is needed to determine the right level of support.
